Personally

  • Begin by filling out an Affidavit of Duplicate Vehicle or Float Ownership (Form DMV-4-TR). In this form, you will need to indicate why you need a duplicate title.

  • You must present your registration card and driver's license.

  • There is a $10 fee for a duplicate name.

  • Take all information to the nearest WV DMV office.

By mail

  • You will need to complete the same form and attach a copy of your registration card and driver's license. Be sure to attach the $10 commission.

  • All information can be mailed to the nearest WV DMV office.
