What documents are required to obtain a Real ID in Puerto Rico?

To meet all federal requirements for these licenses, Real ID applicants in Puerto Rico must provide the following documents:

1. Puerto Rican birth certificate or passport.

2. Fill out the application in pen and legibly.

3. Request a referral from a doctor authorized to practice in Puerto Rico. This certificate cannot be older than 12 months.

4. Request a DTOP-789 certification from an ophthalmologist or optometrist (you must complete this requirement only if your license is in the heavy vehicle category).

5. Original social security card (do not laminate). If you do not have one, you may also file the original Form W-2 Payroll and Tax Statement.

6. Confirmation of the fiscal address with the issuance of no more than two months. For example, an electricity bill, a telephone bill, a water bill, or a bank statement.
